to complete and download the CV Form (AF Form 2662), the Continuing Health Education Program Outline (AF Form 2663) and the faculty disclosure. Email to rose@safcs.org. Abstract packages will not be accepted without the complete package to include Abstract, AF Form 2662, AF Form 2663, and Faculty Disclosure Form.
All abstracts should be submitted for a podium presentation. Abstracts that are not accepted as finalists for the Paul W. Your abstract should be submitted on one page to include the information on page two.
The PWM & Trauma paper must be original work performed by the author and must not have been published prior to the deadline for submission, 4 December 2008. The paper may have been previously presented at another meeting but must not have won any previous award. Chosen presenters will be notified by 24 December 2009 and the completed manuscript for the Paul W. Myers presentation is due 1 January 2010. Presenters must notify SAFCS by 31 December 2009 if they are accepting or declining the invitation to present. Reserve and Deferred status members must arrange for their own expenses, as the Air Force Centrally Funded TDY does not apply.
There will be money awards of $500 and $1000 for the top presenters in the PWM Award, as well as $500 for the top Trauma papers. Competition is open to all members, requires no special application and the Trauma Paper winners have the opportunity to present at the COT competition held annually in August at USUHS.
